Name Change, Marital Status & Payment

• Gazette notification / legal document — for formal change of name
• Divorce/judicial separation decree + affidavit of non-remarriage (if applicable)
• Death certificate (widow candidates) + affidavit of non-remarriage
• Fee payment receipt — SBI Challan or computerised Post Office pay-in-slip
